During your breast augmentation appointment, Dr. Romanelli, Dr. Pincus, or Dr. Layliev will discuss your goals and desired outcome after your surgery, as well as review your medical history. Dr. Romanelli, Dr. Pincus, or Dr. Layliev will thoroughly discuss your breast implant options, including material, size, shape, and placement, and offer a detailed explanation of the surgery. At this appointment, you may try on implant sizers to help you better visualize your desired results. Your surgeon will determine whether there is any asymmetry in your breasts that needs to be addressed, and he may recommend an optional breast lift at the same time as your augmentation.

A mommy makeover is customized to rejuvenate the rest of the body as well—wherever it matters most to you. Our doctors can safely perform multiple procedures together to give patients a greater change with a single surgery and recovery period.
Breast augmentation is performed as an outpatient procedure at Romanelli Cosmetic Surgery. A light general anesthesia is administered by our board-certified MD anesthesiologist. Once the anesthetic has taken effect, the initial incision will be made.
The location and size of the incisions are dependent on several factors. At your consultation, you and your surgeon will determine which of the following options works best for you:
- Periareolar incision: This incision is placed along the lower edge of the areola. This option gives your surgeon direct access to the interior of the breast. The resulting scar is typically not noticeable because it lies along the transition between the areola and the breast skin.
- Inframammary incision: Placed along the breast crease, this incision is the choice of many of our patients. This approach provides the surgeon the most direct route for implant placement. The scar is hidden within or slightly above the fold of the breast.
- Transaxillary incision: This option is placed in a natural crease of the underarm. This incision may limit the size of silicone implants and cannot be used in the future for secondary procedures such as implant exchange or a breast lift as can other incisions.
Breast Augmentation Incision Options
After the incision is made, your surgeon will create a pocket, usually behind the chest wall muscle, for the implants. The implants will be placed in the pocket, along with a long-lasting local anesthetic to minimize post-surgical discomfort. After your incisions are closed and bandaged, a surgical bra or stabilizing band will be placed over your breasts. The entire breast augmentation procedure takes about 1 hour.
With the latest techniques, even silicone gel breast implants can be placed through short incisions.

Recovery & Results
Discomfort and pressure are common within the first 24 to 48 hours after breast augmentation, though the pain is usually manageable with prescribed medication and cool compresses applied to the upper chest. Swelling is also normal, although bruising, if it occurs at all, should be minimal.
Give yourself time to rest and heal at home, and don't rush into activities if you don't feel ready. You may shower or bathe 48 hours after your surgery. Your stitches will dissolve on their own and your follow-up appointment will be scheduled within 2 to 4 days.
Most patients are able to resume light activity and exercise 1 week after their procedures, and more strenuous activity, such as jogging, may be resumed in about 2 to 3 weeks. Refrain from upper-body exercises for at least 6 weeks following your breast augmentation. Nipple numbness or sensitivity is normal, and you should regain normal sensation over the next several months. Your surgeon will provide you with a comprehensive guide to caring for yourself post-procedure.
How long will my breast augmentation last?
Under the right conditions, the results of a breast augmentation are long lasting. However, breast implants should not be considered lifetime devices. Throughout your life, gravity, aging, hormones, weight, and other factors will continue to affect your breasts. A breast lift may be considered in the future to help you maintain a youthful appearance, if needed. Breast implants do NOT have to be replaced every 10 years - that is an internet legend!
What are the risks of a breast augmentation?
As with all surgeries, there is some degree of risk associated with a breast augmentation. Scar tissue, skin wrinkling (sometimes called "rippling"), implant rupture, and loss of sensation are all risks associated with breast augmentation. Using FDA-approved implants, working with a qualified plastic surgeon, and complying with post-op directions will minimize these risks. Dr. Romanelli, Dr. Pincus, or Dr. Layliev will discuss these risks with you before your surgery to ensure you stay fully informed throughout the breast augmentation process.
Who is a good candidate for breast augmentation?
A good breast augmentation candidate has realistic expectations about the outcome of the surgery and is well informed of the preparation, techniques, recovery, and risks associated with the procedure. Good candidates are emotionally stable and physically healthy. People who smoke should quit well in advance of their procedure. As with other cosmetic procedures, you should undergo a breast augmentation for yourself, not to comply with someone else's desires or ideals.
